Shown above is a copy of the Death Certificate of Anastozia Dubiniewicz.  This copy is taken from Ancestry.Com's file of deaths in Ontario Canada from 1869-1932.  Anastozia is my maternal great grandmother.  The medical authorities used one page per three deaths so the first column belongs to a Sarah Dickinson who is of no kin to us.  In the second column is revealed some new and exciting data.  Anastozia's husband was Nicholas Dubiniewicz.  Anastozia was born in Austria (Ukraine) in March of 1856.  Both her parents were born in Austria (Ukraine).  Her father's name was John Bobyk and her mother's name was Annie Garbaezywska Bobyk.    The form also shows that they had been in Canada for 9 years, which puts the immigration date ca 1913.  This was quite a find and helped extend the family tree back several generations and gave more definitive information for further research.
